- name - Required. The name of the person.
- image - Optional. A picture of the person. Shown at 200px width. If not included, will be added to the Category:Memorial No Image.
- birthdate - Required. Date of birth. Full date format looks good: "1st January 2000"
- deathdate - Required. Date of death. Full date format looks good: "1st January 2000"
- mobylink - Optional. A link to the person's mobygames page, if any.
- wikipedia - Optional. The person's wikipedia page name (eg: "This_Is_A_Name"), if any.
- personal - Optional. A link to the person's personal site, if any.
- memorial - Optional. A link to the person's memorial site (eg: one may already exist). If there are more then one, add them to the end of the page instead.
- project_state - Required. This will be set according to the editor, to determine what memorials have adequate information and biography/obituary entries and those that are in progress or haven't got much info.
- 0 - Will be added to Category:Memorial Started - This is to mark the memorial for this person has only got basic information but has been started.
- 1 - Will be added to Category:Memorial Incomplete - This is to mark the memorial for this person has got some information but is in some way incomplete and needs work.
- 2 - Will be added to Category:Memorial Waiting On Info - This is to mark the memorial for this person has got some information and that information is being waited on from a 3rd party.
- 3 - Will be added to Category:Memorial Complete - This is to mark the memorial for this person has been completed to a standard that requires no more active input.
Additionally the page will be added to the Memorials category.
